New Patient Special
Your initial appointment will consist of a consultation. Each case is unique and individualized so the doctors at Premier Dental take detailed records of your existing dental and medical conditions to create a foundation for a plan that is safe, effective, and long lasting. Records can include radiographs, models, extra-oral, and intraoral photos.
Your initial appointment will also require you to complete a Patient Information Form to address pertinent personal information and medical history. Please allow for extra time to complete this form or take a moment to complete the Online Registration available on this website. The Registration Form will automatically be forwarded to our office. Upon arrival at your scheduled appointment time and date, you will be provided with the opportunity to review and sign a printed version of the Patient Information Form.
Please assist us by providing the following information at the time of your consultation:
- Any previous x-rays if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring the necessary completed forms and dental cards. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med).
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. You may have your previous dental provider email digital x-rays to our office manager at [email protected]

At Premier Dental we understand that dental emergencies occur morning, noon, or night. That's why Dr. Aronson and Dr. Travan are available 24/7 - ready to meet the needs of their patients when they need them the most.
If you find yourself in a dental emergency that cannot wait until business hours, call our office, wait for the prompts for emergency service, and the doctor on call will return your call as soon as possible.
Tooth pain doesn't wait, so why should you? Half of the anxiety people experience is due to feeling that they are bothering their doctor. At Premier Dental, your health is more important than whether a phone rings in the middle of the night. Alleviate the anxiety, and you alleviate a lot of the pain.

We deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement and submit your insurance claims electronically. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but this must be done prior to the actual procedure.
